Use Forms to ensure necessary documentation is in place to protect both the gym and its members, while also facilitating a smooth on-boarding process for new members.

Digital Forms are automatically saved to Communication Log

To view forms;

• Go to Member Details > Communication tab
  • Select Questionnaires or Contracts
    

    

  • From here, you can select to view Waivers, Agreements, Questionnaires and Contracts
