Forms and Waivers

Use Forms to ensure necessary documentation is in place to protect both the gym and its members, while also facilitating a smooth on-boarding process for new members.

  • Add Forms - Create your;
    • Membership Contracts - agreement between a gym and a member, typically includes details such as membership type, duration, fees, payment terms, terms and conditions, cancellation policies
    • Terms and Conditions - rules and policies that govern the use of the gym’s facilities and services
    • Cancellation Terms – conditions that govern cancellation of gym membership by either party
    • Waivers - risks and responsibilities associated with using the gym equipment and facilities
    • Questionnaires - used by gyms to collect information from members or potential members
  • Attach Forms to Class / Service - Automatically prompt member to complete attached forms upon booking
  • Attach Forms to Membership - Automatically prompt member to complete attached forms upon purchase
  • Require Signature to Complete Membership - Automatically prompt member to sign attached form upon purchase
  • Enable Questionnaire After Sign-up - Automatically prompt member to complete questionnaite upon sign-up
  • Save / View Member Forms - Store and view all completed forms
